IgA Pyroglobulin , Hyperviscosity Syndrome and Coagulation Abnormality in a Patient With Multiple
نویسنده
چکیده
A 48-yr-old woman with a bleeding diathesis and complaining of blurred vision, headache, paresthesias, and vomoting was diagnosed as having lgA, kappa type multiple myeloma. Markedly increaesed serum viscosity was noted. After plasmapheresis the signs and symptoms of high serum viscosity disappeared. Pyroglobulin was found in the M protein. The M protein had a prominent 15.5S peak and smaller 17.0 and 20S peaks, which decreased to a 6.7S peak after in vitro reduction with 0.1 M 2-mearcptoethanol, 2-mercaptoethylamine hydrochloride or glutathione. The serum viscosity was decreased by treatment with these reducing agents. Pyroglobulin formation was presumably a noncovalent intermolecular reaction, as it was inhibited by 5.0 M guanidine hydrochloride, acidification and alkalin ization, but not by reduction with sulfhydryl reagents. The M protein also had an antithrombin activity causing interference with the conversion of fibrinogen to fibrin and the phenomenon of gelification noted on blood clotting.

Rib plasmacytoma and IgA multiple myeloma with hyperviscosity syndrome.
Solitary bone plasmacytoma (SPB) can progress to generalized myeloma if not treated early. The elderly population is increasing and delays in diagnosis of plasma cell malignancies are frequent among them. Hyperglobulinemia of multiple myeloma (MM) plays a role in hyperviscosity syndrome (HVS). A 65-year-old woman with hypertension and diabetes mellitus was admitted due to loss of appetite, musc...
متن کاملErroneous results with routine laboratory testing for immunoglobulins due to interference from circulating immune complexes in a case of hyperviscosity syndrome associated with autoimmune disease.
A patient with the hyperviscosity syndrome exhibited very high concentrations of intermediate to small circulating immune complexes (CIC), involving 40-50% of the IgG present, with IgG rheumatoid factor activity. We demonstrate that precipitation of CIC by polyethylene glycol in the reaction mixture caused interference with nephelometric methods for measuring IgM and IgA, and that failure of im...
متن کاملA case of rheumatoid hyperviscosity syndrome with characterization of the serum immune complexes.
The clinical and immunologic data in a case of rheumatoid arthritis complicated by serum hyperviscosity are reported. The elevation in the serum viscosity was accompanied by the presence of intermediate immune complexes which were partially purified from the patient's serum by gel filtration chromatography. These complexes had a molecular weight of approximately 510,000 daltons and contained po...

A 45-year-old man with polyclonal hypergammaglobulinemia (gamma globulins, 102 g/L) had a serum relative viscosity of 13 nu but did not manifest clinical signs of hyperviscosity syndrome (e.g., retinopathy, bleeding diathesis, and neurological alterations), except for fatigue and anorexia. In contrast with other patients with polyclonal hyperviscosity reported so far, this patient did not have ...
